Case Management

Because some workers compensation injuries require more attention for prompt return-to-work, a nurse case manager will:

• Keep the employer informed about changes in injury status.
• Schedule all authorized referrals within a specified time.
• Alert you if an injured employee is non-compliant with appointments.
• Keep the employee, employer and the payer informed.
• Complete and maintain all forms per state requirements.
• Formulate a return-to-work action plan.
• Be available to answer all of your questions.

Employee Work Status Service

With Work Partners, all injuries are monitored beginning with their initial visit to the provider and are followed through the course of their treatment to ensure that no one falls through the cracks. The goal of our service is to help maintain control of injured workers and return them to work as soon as medical treatment allows. Services include:

• Provider Reports faxed within 24 hours of initial provider appointments.
• All specialist visits within 4 days of referral.

• Verification that all referrals stay within the Work Partners Provider Network.
• Alerting the employer if an injured employee is non-compliant.
• Monthly tracking reports.
