Human Resources — Your First Day of Employment

Representatives of Human Resources will provide your initial new hire documents and collect required documentation from you. On your first day of Orientation, you will be asked to supply the following information:

  • Driver’s License or State Issued Picture ID
  • Birth Certificate or Social Security Card
  • Voided Check for Mandatory Direct Deposit

During the orientation process, new employees will be required to submit various forms and paperwork. You are encouraged to click on the forms below, complete them, print them out, and bring them with you to orientation on Monday morning.

Please arrive 15 minutes early to accommodate time for locating the conference center, parking, registration, and refreshments.

Employee Health — Preparing for Your Health and Safety at Work

As a new employee, you will be required to complete an Employee Health visit prior to your first day of employment. Human Resources will arrange the appointment date and time with you. This appointment lasts approximately 45 minutes.

Please plan to arrive 15 minutes early for your appointment time with the following information:

  • List of any over the counter medications taken in the past two weeks
  • Bottles for any prescription medications you are currently taking
  • Immunization records
  • Government Issued or School issued Photo ID
  • Social Security Card:

Parking on New Employee Orientation Day:

Free parking is available in the employee-designated areas of the Jackson-Madison County General Hospital campus. You will park in the visitor Parking Garage #1 (corner of Forest & Summar) on the roof top level. For driving directions and map to orientation, click here.

To locate the J. Walter Barnes Conference Center, you can walk into the hospital on either the blue or orange levels. At the end of the hallway, turn left and walk until you see the escalator. Take the escalator up to the main lobby. The conference center is on your left.

What to Wear...

Professional/casual business attire or uniform (no jeans, capri pants, flip flops, sweat pants, shorts, or exposed midriffs allowed). Consider wearing layered clothing since the room temperature may not be comfortable for each individual. No tattoos should be visible as well as bare middriffs and/or body piercings. Women are allowed a maximum of two ear piercings in each lower lobe.
