Direct Access Colonoscopy Form

Direct Access Screening Colonoscopy allows a patient age 75 or younger who has met the screening criteria to bypass the need for a separate consultation with a GI physician and be scheduled directly for a screening colonoscopy.  A patient’s primary care physician can refer you to this program, or if your insurance does not require a referral, you can directly call West Tennessee Gastro at 731-424-1001 or complete the online form.

A nurse coordinator will contact you by telephone once you have completed the short medical history screening.  The nurse coordinator will use the information provided in the health questionnaire to determine if you are a candidate for the Direct Access Screening Program.  If you are a candidate, the nurse coordinator will schedule your screening exam directly at Skyline Endoscopy Center and provide you with the instructions for the procedure.  If you are not a candidate, the nurse will help you schedule a consultation visit with the GI physician of your choice.

Our gastroenterologists will meet with you at the time of your procedure.  After the procedure is completed, they will talk to you about your exam results.  Any pathology results will be mailed to you and your primary care physician, along with any recommended follow-up.

By providing our patients with Direct Access Screening Colonoscopy, we hope this will help you to arrange your colon cancer screening easier and more conveniently.

American Cancer Society Screening Age Recommendations:
Beginning at age 50, both men and women should follow one of these testing schedules:

  • Flexible sigmoidoscopy every 5 years, or
  • Colonoscopy every 10 years, or
  • Double-contrast barium enema every 5 years, or
  • CT colonography (virtual colonoscopy) every 5 years
  • Yearly guaiac-based fecal occult blood test (gFOBT), or
  • Yearly fecal immunochemical test (FIT), or
  • Stool DNA test (sDNA) every 3 years
