Thank you for choosing Sports Plus Rehab Centers! The following information will guide you through your first day of therapy and the process to begin your Physical or Occupational Therapy Rehabilitation program with us.

If you’re a new patient, here’s what you need to know for your first visit:

Arrival Time

Please arrive 10-15 minutes early to complete your new patient registration forms. Our staff will greet you and review the forms. They will schedule all of your appointments needs.

What to Bring/Prescription for Treatment

You will need to bring your order/prescription from your physician, a list of medications and allergies, and a photo ID.

Insurance Verifications

Please bring your insurance card for our staff to verify your insurance plan and eligibility.

What to Wear

We recommend that patients wear loose-fitting exercise clothing and tennis shoes for comfort.

Meet Your Therapist

You will meet with a physical or occupational therapist who will evaluate your injury/condition and generalized pain. The therapist will discuss your individualized treatment plan, work with your physician and set goals for your specific need.

How Long Will It Take?

Your initial visit will take an hour or more.
