We are happy that you and your family have chosen our family to serve your therapy needs. All new clients are asked to complete a Client Information Packet upon their first visit.
Parents and caregivers, for your convenience and to help expedite the need to gather valuable information, we have made our new client information packet available prior to your initial visit. Please download the new client information packet and bring this with you 15 minutes prior to your visit.
In order for your child to receive an initial evaluation or ongoing therapy services from the Therapy and Learning Center, we will need (2) items from your doctor:
1. Referral form
Your doctor may already have our referral form. If they do not, we would be glad to provide that to them. Click here to Access the referral form. Your doctor can fax that information to us at 731-660-6145. Once we receive this information we will contact your family to schedule the appropriate appointments.
