About

Prime Care Selmer is a novel coronavirus (COVID-19) testing site. ALL patients are asked to call ahead for any treatment (731-645-7932), and clinic staff will give every patient specific instructions to follow, including pre-screening by phone.  At that point, the patient will either be:

  1. Instructed to remain home and self monitor, or
  2. Instructed to come to this location for evaluation and testing.

All patients who present to our clinic locations will be required to wear a mask and clean their hands prior to entering the clinic. Restrooms and sanitizer are available at clinic entrances.

Patients will only be tested if they meet the CDC testing qualifications, which are:

  • Fever

  • Cough

  • Shortness of Breath

  • Close contact with a suspected or laboratory-confirmed COVID-19 patient

West Tennessee Medical Group Prime Care Selmer is recognized as a Level 3 NCQA Patient-Centered Medical Home. The patient-centered medical home (PCMH) is a model of care that puts West Tennessee Medical Group patients at the center of care. Research shows that PCMHs improve quality and create a better experience for West Tennessee Medical Group patients.
