Please reach out to us at (731) 541-5000 to obtain information about our clinical specialists and services; schedule and confirm appointments for your patients; receive assistance in the resolution of any service-related issues, and connect with West Tennessee Healthcare specialists.
When contacting us to refer your patients, please provide the following information:
- Patient demographic information
- The reason for visit or referral
- Any special needs that the patient will need to be facilitated once the patient arrives
- The patient’s insurance information, if available
Once this information has been provided, West Tennessee Healthcare will work with your patient at their convenience to complete the registration process and to schedule an appointment.
